Pelvic Abscess occurring after Transvaginal-Ultrasound Guided Oocyte Retrieval in IVF Program.

Abstract
Transvaginal ultrasonographically guided oocyte retrieval has become a standard method in most IVF centers that proved to be practical and efficient procedure in large series of IVF literatures. Complications such as anesthetic accidents and
possible
injury to internal organs often resulted from laparoscopic route have been reduced dramatically by using this transvaginal route. However, we experienced a few, but grave complications after oocyte retrieval; pelvic abscess, possibly requiring
exploratory laparotomy and long-term hospital admission.
The purpose of this study was to evaluate the incidence and clinical courses of pelvic abscess after oocyte retrieval via transvaginal route. We reviewed 1576 cycles that have undergone IVF-ET from Jan. 1, 1994 to Dec, 31, 1995.
@ES The results were as follows; @EN The pelvic abscess occurred in 4 cases among, 1576 cycles (0.25%), every patient complicated by pelvic abscess had at least one risk factor, such as hydrosalpinx, endometriosis, and history of previous PID.
Among
these patients, one suffered from life threatening sepsis that recovered with surgery and vigorous administration of antibiotics while others were promptly recovered without sepsis by conservative treatment and/or appropriate surgical
intervention.
In conclusion, although oocyte retrieval via transvaginal route is a safe method, if pelvic abscess occurs, aftercoming results can be a dangerous condition. Therefore, application of aseptic technique, thorough vaginal preparation and
prophylactic
antibiotics should be done during IVF procedure. Furthermore, close following up after IVF procedure should be performed in high risk group, and if pelvic abscess develops, adequate and prompt management should be done as soon as possible.
